Transcript Request

Complete the transcript request form to receive an official Seminary transcript. Official transcripts will be sent to the location provided on the form. Return the completed form to the Registrar’s Office at registrar@csl.edu, or by mail. Official transcripts can also be requested and picked up in the Registrar’s Office.
